Association between Albuminuria and Severity of Chronic Obstructive Pulmonary Disease: The 8th Korea National Health and Nutrition Examination Survey (2019)

Abstract
Background: Chronic obstructive pulmonary disease (COPD) is a chronic disease featured by an irreversible airflow limitation. Albuminuria is a possiblepredictor of cardiovascular disease (CVD), a common comorbidity in patients with COPD. This study aimed to investigate the relationship betweenCOPD and albuminuria based on the 8th Korea National Health and Nutrition Examination Survey (KNHANES).

Methods: We used data from the 8th KNHANES in 2019 and included 2681 adults aged over 40 years. Odds ratio (OR) and 95% confidence interval (CI)were calculated using complex sample logistic regression analysis to find the association of albuminuria (albumin-creatinine ratio ¡Ã30 mg/g)according to the presence of COPD and the severity of airflow limitation.

Results: Mean age of study population was 57.8 (standard deviation, 11.2). Of the participants, 236 (8.8%) were found to have albuminuria. Participantswith COPD had higher OR of albuminuria (OR, 1.71; 95% CI, 1.05?2.79) compared with those without. Even after adjusting for confounding variablessuch as sex, age, diabetes, hypertension, smoking status, high risk drinking, history of CVD, estimated glomerular filtration rate, body mass index,and hypercholesterolemia, according to the severity of airflow limitation, The OR of albuminuria was higher in the moderate (OR, 1.87; 95% CI, 1.06?3.31) and severe groups (OR, 4.96; 95% CI, 1.56?15.72) than that of the participants without COPD.

Conclusion: Albuminuria is associated with COPD and increase in the severity of airflow limitation in Koreans.
